Arizona Long Term Care Services (ALTCS)
The Arizona Long Term Care Services (ALTCS) tribal case management program is an integrated health plan formed by an Intergovernmental Agreement between the Arizona Health Care Cost Containment System (AHCCCS) and the San Carlos Apache Tribe. It provides comprehensive long-term care case management services to tribal members enrolled in ALTCS. The ALTCS elderly and physically disabled long-term care plan is dedicated to individuals of all ages who require ongoing medical and daily support. Eligibility criteria is determined based on medical necessity and financial information.
ALTCS Member Services and Healthcare Benefits
Tribal ALTCS members are eligible to receive a wide range of essential health and daily living support services, including:
- Attendant care (in-home personal care attendant)
- Durable medical equipment and medical supplies
- Emergency Alert System support
- Home delivered nutritious meals
- Home modification to create a handicapped accessible home
- Hospice services (community and institutional care)
- Skilled Nursing facility and Assisted Living services
- Physical, occupational, and speech therapy
- Non-emergency medical transportation to medical appointments
Tribal Case Management and Application Process
Tribal ALTCS members are assigned dedicated Case Managers who assist them in choosing the personalized care services they need. The primary goal of ALTCS is for members to remain healthy, happy, and independent. Case Managers coordinate medical care, arrange medical supplies, and work with certified agencies to provide in-home care services. Patients may choose a qualified family member or a professional healthcare provider, and decide whether to live at home or receive care in an assisted living facility.
